Chronic Disease Management Programme

The Medisave for Chronic Disease Management Programme (CDMP) aims to improve care for patients with chronic diseases, resulting in better health outcomes and lower long term healthcare costs.

You can use your Medisave for the following chronic conditions:

  • Diabetes
  • Hypertension
  • Lipid Disorders
  • Stroke
  • Asthma
  • Chronic Obstructive Pulmonary Disease
  • Major Depression
  • Schizophrenia
  • Dementia
  • Bipolar disorder
  • Osteoarthritis, Benign Prostatic Hyperplasia, Anxiety, Parkinson’s Disease and Nephrosis/Nephritis (from January 2014)

These are the steps for using your Medisave for Vaccination at our clinic:

We will need to certify in the Medisave Authorisation form that you suffer from one of the chronic conditions in the above list.

For each bill, patients will only need to pay 15% of the bill. Medisave can be used to pay for the remaining amount. This is regardless of whether the bill is for a one-off visit or a package.

Annual Withdrawal Limit:

Withdrawals will be subject to an annual outpatient withdrawal limit of $400 per Medisave account with effect from 1 Jan 2012. Patients can also use the Medisave of their immediate family member(s)* to pay for their treatment, up to a limit of $400 per year per account. A maximum of up to 10 accounts may be used.

* Immediate family member refers to spouse, child, parents. Grandparents who are citizens or permanent residents of Singapore may use the Medisave of their grandchildren as well.

You may use your Medisave for the following approved vaccinations:

1. Outpatient Vaccinations (under the National Childhood Immunisation Schedule)

  • Pneumococcal vaccinations (for children under 6 years of age)
  • Human Papillomavirus (HPV) (for females aged between 9 – 26 years of age)
  • Hepatitis B vaccination
  • Measles, Mumps and Rubella (MMR), Tuberculosis (BCG), Diphtheria, Pertussis & Tetanus (DTaP/Tdap), Poliomyelitis, Haemophilus Influenza Type B (Hib)

2. Outpatient Vaccinations

  • Influenza and pneumococcal vaccinations (for persons with higher risk of developing influenza-related complications and severe pneumococcal disease respectively)

These are the steps for using your Medisave for Vaccination at our clinic:

We will need you to sign a Medisave Authorisation form for yourself or your family member.

There is no deductible or co-insurance. Medisave can be used to pay for the full cost of the vaccination up to the Annual Withdrawal limit.

Pioneer Generation

The Pioneer Generation Package provides healthcare benefits for all Pioneers, for life. All Pioneers will receive special subsidies at CHAS General Practitioner (GP) and dental clinics.

Nuffield Medical is a participating CHAS outpatient GP clinic.

The Community Health Assist Scheme (CHAS) is a scheme by the Ministry of Health (MOH) that enables Singapore Citizens from lower- and middle-income households to receive subsidies for medical and dental care at participating General Practitioners (GPs) and dental clinics near their homes.

Singapore Citizens who qualify for CHAS will receive an individual blue or orange Health Assist card. Health Assist cardholders will also enjoy subsidised referrals to Specialist Outpatient Clinics (SOCs) located at Public Hospitals or National Dental Centre when required.

For patients with chronic conditions, CHAS complements the Chronic Disease Management Programme (CDMP), which allows for Medisave to be used for outpatient treatment (for the same set of chronic conditions) covered under CHAS. Besides enjoying CHAS subsidies for treatment of their chronic conditions, patients can also tap on their Medisave to defray part of the cost of these treatments. The Medisave withdrawal limit is $400 per Medisave account, per year.

From 1 January 2014, all Singapore Citizens (regardless of age) who meet the household monthly income per person of $1,800 and below can qualify for CHAS. For instance, a family of four living at the same address with a total gross household monthly income of $7,200 (household monthly income per person is $7,200 ÷ 4 = $1,800) will qualify for CHAS, provided that they are all Singapore Citizens.

The colour of the Health Assist card indicates the subsidy tier that you are entitled for, either CHAS Subsidy Blue or Orange tier.
